How it Works

When a loved one is living in a nursing home or assisted living facility, one of the biggest concerns families have is simple:

Care Oversight Visits provide independent, nurse-led check-ins designed to give you a clear, objective picture of your loved one’s day-to-day care. Visits are conducted at varied days and times to reflect real conditions, not just what is seen during scheduled or expected visits. We try to go during "off times" when staffing is likely to be low, administration staff are not around, and we can spot hidden issues and problems.


During each visit, a qualified nurse team member will look at your loved one’s overall condition, environment, and general care indicators. This includes observations related to cleanliness, positioning, responsiveness of staff, engagement, and any visible changes or concerns. Following each visit, you receive a detailed written report, outlining observations, clinical insight, and clear recommendations so you can make informed decisions and take appropriate action if needed.


This service is designed to provide transparency, peace of mind, and an added layer of protection for families who cannot be present on a regular basis. It is especially valuable for those living out of the area or for families who have concerns about the consistency or quality of care being provided.

What we look for: 

  • Resident hygiene and appearance
  • Positioning and mobility
  • Responsiveness of staff
  • Safety risks (call light, fall risks, accessibility)
  • Nutrition and hydration indicators
  • Overall environment and cleanliness
  • Changes in condition or behavior
  • Any other clinical issues or concerns that are brought to our attention during the visit
  • Timing visits when administration is likely to be gone, and staff is likely to be short to see how care is being provided
  • What's really going on when no one is around!
  • What we report on: 

  • Hygiene, positioning, and general overview of the environment
  • Accessibility of essential items (call light, water, personal belongings)
  • Safety concerns (fall risks, clutter, equipment placement)
  • Staff presence, responsiveness, and interaction style
  • General response times (when observable)
  • Nutrition and hydration indicators (meal intake, accessibility)
  • Medication-related observations (visible concerns only)
  • Emotional well-being and signs of distress or isolation
  • Compliance with basic care expectations (as observable)
  • Notable concerns, inconsistencies, or red flags
  • Clear clinical insight and recommendations for next steps
  • Recommendations for hospice or additional support services, when the time comes
  • Reporting to state oversight and state agencies if necessary (we take the burden off of you)

    Pricing is autobilled through our secure portal each month and you receive an invoice and receipt for transparency.
